Basic Model
Use this version with Microsoft Excel™ or OpenOffice™ Spreadsheet if you have seven or fewer medicines and supplements.

Obtain a Comprehensive Medicine Review
One of the easiest ways to reduce the risk of an Adverse Drug Event is to ensure each of your healthcare providers has your medical information up-to-date. Obtaining a periodic review by an experienced healthcare provider can also be helpful –numerous studies have indicated that pharmacist-provided medication therapy reviews and consultations result in reductions in physician visits, emergency department visits, hospital days and overall healthcare costs.
A comprehensive review helps ensure you’re receiving safe and effective treatments and are aware of the risks and benefits of your treatment options. A review concludes with a summary of any recommendations made and an updated list of your current treatments, such as a completed template, as shown above. When a change in treatment or additional monitoring appears appropriate, you'll receive recommendations written to your prescriber, which you may then send to him or her, and/or discuss during your next appointment.
If you have health insurance, consider checking with your plan’s administrator for assistance in finding a healthcare provider skilled in Medication Therapy Management; many plans offer these services and will cover some or all of the costs.
